KFL&A Public Health collects and maintains the immunization records of all school-aged children within the KFL&A area in accordance with the Immunization of School Pupils Act (R.S.O. 1990) (ISPA). Children aged 4 to 17 years must be immunized following Ontario's Immunization Schedule and all students must provide proof of immunization to KFL&A Public Health for these diseases:
If a parent or guardian chooses to exempt their child from immunization for non-medical reasons, they will be required to complete an educational module at KFL&A Public Health and have notarized a Ministry of Health and Long-Term Care Statement of conscience or religious belief - Immunization of School Pupils Act exemption form. The parent or guardian must provide, one form per child attending school, the original form to KFL&A Public Health. The process to complete the educational module will require 45 minutes to 1 hour of time. The education module addresses the risks and the benefits of immunization, how vaccines work, herd immunity, vaccine safety, and immunization schedules. Each year, KFL&A Public Health reviews the immunization records of all elementary and secondary school students to ensure that they meet the ISPA requirements. Parents will receive letters from KFL&A Public Health if their child is missing any of the required immunizations. Children who do not have an up-to-date immunization record or valid exemption on file at KFL&A Public Health may be suspended from school for up to 20 school days or until the necessary information is received. Parents or caregivers are responsible for reporting to KFL&A Public Health each time your child receives an immunization. Your health care provider does not report these records. Grade 7 school-based immunization programs |
KFL&A Public Health provides school-based immunization clinics for Grade 7 students for hepatitis B, meningococcal, and human papillomavirus (HPV) immunizations. As of January 1, 2017, KFL&A Public Health no longer provides non-publicly funded (paid) vaccines. Please contact your health care provider for more information on how to receive the following vaccines if you fall outside the publicly funded criteria.
Students require consent from a parent or guardian to receive these immunizations at school. Immunization information and consent forms are sent home through your child's school in early September. Please sign and return your child's consent form to the school within one week. You can also fill the consent forms out online:
