Dear Fellow Boater;
We, as a boating community, are always striving to make our time on the water as safe as possible. In 1994 a survey was done to gather information from local boaters and sailors regarding their attitudes and behaviours towards safe boating practices. The survey is being repeated to determine whether there have been any changes. Name of Marina is supporting the collection of this information from local boaters. The survey is being conducted by KFL&A Public Health. Enclosed you will find a questionnaire and explanatory letter from Karen Scott, a research associate at KFL&A Public Health, who is the project coordinator for this survey. Your participation in the study is voluntary. The questionnaire is completely anonymous and your responses will be kept confidential. Your name and address have not been given out to KFL&A Public Health; I am mailing this out myself to ensure your privacy. No organization, including Name of Marina, will have access to your individual responses. Reports will combine the responses of all participants. A summary report of the findings will be made available to you. I do hope you will take the time to complete the questionnaire and return it to Karen Scott c/o KFL&A Public Health. Dear Boater or Sailor;
As mentioned in [name of marina director]'s letter, we are repeating a survey of Greater Kingston area boaters, which was first conducted in 1994.
The purpose of the survey is to gather information that can be compared to the information collected in the first survey. This comparison will be useful in planning health promotion and injury prevention programming.
The following points explain how the survey is being conducted:
Whatever your experience and views, your opinion counts. Please use the enclosed stamped envelope to return the questionnaire to KFL&A Public Health.
We would like to invite you to enter our draw for a Mustang Classic Inflatable PFD or a Gift Certificate for one Boat Pro (Pleasure Craft Operator Card) Seminar. If you wish to be entered, complete the postcard and mail it in a separate envelope from your completed questionnaire. We will make the draw for winners at the end of May (2010) and will inform you if you have won a prize.
We hope that you will take the time to participate in this project in order to ensure that accurate information is obtained. The results of the survey will be used by KFL&A Public Health and other organizations interested in the issue of boating safety.
