Make a request for your health or general record 

If you would like to get a copy of your personal health record or a general record from KFL&A Public Health, please complete a health or general record request. For a copy of your immunization record, please call 613-549-1232 during business hours. 


KFL&A Public Health is committed to respecting your privacy and protecting your personal health information. Personal health information is being collected under the authority of Health Protection and Promotion Act, Municipal Freedom of Information and Protection of Privacy Act, Personal Health Information Protection Act, and other legislated Acts, as required. For more information on the agency’s privacy policies.

The employees of KFL&A Public Health collect personal health information about individuals directly from them or from the person acting on their behalf. Employees collect only as much personal health information as is needed to do their jobs. It may be necessary to collect personal health information about individuals from other sources if consent to do so has been given and if the law permits.

KFL&A Public Health collects, uses, and shares personal health information to:

  • provide public health programs and services,
  • plan, administer, and manage programs and services,
  • conduct quality improvement and risk management activities,
  • compile statistics to assess the health of our population,
  • comply with legal and regulatory requirements, and
  • fulfill other purposes permitted or required by law.

Individuals may withdraw consent for some of the above collections, uses, and disclosures, subject to legal exceptions.

Purpose

KFL&A Public Health has a responsibility to protect the personal health information of the people to whom it provides services. This policy provides guidelines for the collection, use, and disclosure of personal health information in accordance with the Personal Health Information Protection Act (PHIPA).

Policy

The right of every individual to privacy shall be recognized and protected to the greatest extent possible, consistent with the public interest and legislated requirements.

Kingston, Frontenac and Lennox & Addington Public Health shall collect, use, and disclose personal health information and provide access to personal health information in accordance with PHIPA and other applicable legislation.

The Director, Corporate Services shall be the Privacy Contact Person for KFL&A Public Health.
