KFL&A Public Health has several programs, clinics, and services that health care providers can refer patients to promote health. 

Healthy Babies Healthy Children (HBHC) or Nurse-Family Partnership (NFP) program

The Healthy Babies Healthy Children program is a free home visiting program that helps children get a healthy start in life by helping expectant parents and families with children up to transition to school through:

  • Screening and assessments to identify any needs that could affect a child's healthy development.
  • Supporting new parents.
  • Helping to find community programs or resources on various subjects such as breastfeeding, parenting, nutrition and health services, and family literacy programs.

The Nurse-Family Partnership (NFP) program is a free home visiting program where a public health nurse visits young, first-time parents during their pregnancy and the first two years of their child’s life. The program can help support parents to have a healthy pregnancy, a healthy baby, and continue to meet their education and work goals.

Eligibility for the Nurse-Family Partnership program includes individuals:

  • 24 years of age or younger.
  • Pregnant with their first child or first-time parenting.
  • Pregnant 28 weeks or less.
  • Experiencing limited supports or resources (socioeconomic challenges).

If participants are not eligible for the NFP program, they may be eligible for the HBHC program.

To access the Healthy Babies Healthy Children program or the Nurse-Family Partnership program, patients can self-refer. Referrals may also be made on their behalf by a community service or health care provider. 

How to refer to the Healthy Babies Healthy Children program or the Nurse-Family Partnership program:

  • Send a completed Referral Form for Service Providers, or Self-Referral Form to KFL&A Public Health.
  • Call our Parenting in KFLA phone line at 613-549-1154.

Food for You, Food for Two prenatal program

Food for You, Food for Two is a free prenatal program for pregnant people and their support person(s). Participants will connect with other pregnant people, learn about food, nutrition, pregnancy, and taking care of a baby.

If you have a client that would benefit from this program, please complete a referral form.
