Record searches
KFL&A Public Health will conduct a search to locate all records related to the operation of a premise for an indicated property. There is a $150 fee.
Note: property owner must provide permission to release their personal information.
Application instructions
- Print and complete the Record Search Application and save to your computer. You will need to upload this to your online payment form in step.
- When printing, please ensure the "scale, fit to page" option is turned off.
- Complete the Record Search Payment form, and upload the completed forms as instructed.
- Have your credit card ready for payment. If you prefer to pay with debit or credit card in-person, please visit a KFL&A Public Health office.
