Routine immunization reporting
In Ontario, health care providers do not automatically report immunizations to their local public health agency. Parents and guardians are responsible for reporting their child's immunizations to KFL&A Public Health to keep records up-to-date per standards of the Immunization of School Pupils Act.

- Submit immunization records online with Immunization Connect Ontario (ICON)
- Bring a copy of the vaccination record to a KFL&A Public Health office during regular business hours.
- Fax a copy of the vaccination record to 613-549-0985 (confidential fax).
- Mail a copy of the vaccination record to KFL&A Public Health - Attention Immunization Team, at 221 Portsmouth Avenue, Kingston ON K7M 1V5

Online
- You can view immunization records online with Immunization Connect Ontario (ICON)
- You will need a personalized Ontario Immunization Identification Number (OIID). To receive your OIID, please call KFL&A Public Health at 613-549-1232, or 1-800-267-7875.
In-person
You can request to pick up immunization records in-person at a KFL&A Public Health office during regular business hours.
External methods
- Contact your family doctor or previous clinics where you have been immunized.
- When physicians retire or a medical practice changes hands, you may be able to get help locating records by contacting the Advisory Services Department with the College of Physicians and Surgeons at 1-800-268-7096, ext. 603.
- Look at home in your old papers, including baby books and school or camp forms. If you are an adult, don't forget to ask your parents or guardians if they still have your childhood records.
- Contact schools, colleges, or universities that you or your child has attended.
- Contact previous employers, including the military.
Reporting COVID-19 immunizations received outside of Ontario or Canada
- This reporting form is for residents and members of the Canadian Armed Forces in Kingston, Frontenac, and Lennox and Addington counties ONLY

- Mail a copy of your COVID-19 immunization record to KFL&A Public Health - Immunization Team, or

Due to the sensitivity of Immunization Records as Personal Health Information under the Personal Health Information Privacy Act (PHIPA), immunization records cannot be received or sent via e-mail.
